Guided by the core philosophy of “Light of Life, Gentle Guardianship” we developed the L.I.G.H.T. Care Model, a comprehensive framework designed to deliver continuous, safe, and high quality care for high risk mothers and infants across the full care continuum-from antenatal and intrapartum care to postpartum support and early intervention services.
The L.I.G.H.T. Care Model integrates five key dimensions: Linking, strengthening parent–infant bonding and nurse–patient relationships; Innovation, incorporating advanced technologies and innovative care practices; Guardianship, ensuring physical and psychological safety with proactive risk prevention; Holistic, fostering interdisciplinary collaboration and early therapeutic intervention; and Tracking, enabling long term follow up and continuity of care.
A total of 11 innovative initiatives were implemented, including the clinical application of four newly patented and National Innovation Award recognized medical devices-a safety breastfeeding pillow, an improved neonatal identification ankle band, an intravenous catheter protective device, and a quantitative neonatal blood sampling device. These innovations are complemented by structured assessment processes, such as SPARC-T holistic assessment, postpartum depression screening, and evaluations of feeding difficulties in preterm infants, ensuring both precision and human centered care.
To further enhance continuity, LINE Official Accounts (LINE OA) are utilized to track the growth and development of preterm infants up to two years of age, establishing a robust interdisciplinary long term follow up mechanism. Through integrated technology, standardized assessments, and collaborative care pathways, the L.I.G.H.T. Care Model significantly enhances safety, efficiency, and care quality for high risk mother-infant dyads, demonstrating a distinctive nursing innovation that balances clinical excellence, compassion, and patient safety.
