In response to the growing challenges of an aging society and care gaps for elderly living alone, China Medical University Hospital launched the “Elderly Living Alone Care Project” in 2014. Centered on Taichung’s mountain and Tun districts and Xinyi Township in Nantou County, the project integrates cross-disciplinary teams, community engagement, and precision care to build a holistic support system. Through the “Bridge Builder Model,” it connects elderly individuals with medical, social, technological, and community resources. Services include comprehensive geriatric assessments, remote care, case management, psychological support, and community participation. AI video monitoring and a 24-hour toll-free hotline enhance safety and responsiveness. Using PDCA cycles, the team continuously improves service quality and demonstrates strong execution and human-centered care. The project has significantly reduced loneliness, disability, and dementia risks, while enhancing dignity and well-being. With an SROI of 4.25, this model offers a scalable and replicable solution for elderly care in Taiwan, embodying the ideals of aging in place and holistic care.
